We are seeking a talented Registered Nurse to join our team. As a Home Health and Hospice RN, you will be responsible for planning, organizing, and directing home care services. You will have the opportunity to work in a community health setting, providing education and support to individuals and families within their homes and communities.

* Job Summary:
** The Home Health and Hospice RN plans, organizes, and directs home care services and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
*
* Duties & Responsibilities:
*** Develop and initiate the plan of care in partnership with the patient, representative, and caregiver(s)
* Provide preventative and rehabilitative nursing procedures as ordered by the physician
* Offer patient, caregiver, and family counseling
* Deliver patient and caregiver education
* Prepare clinical notes
* Communicate with the physician and other healthcare practitioners related to the home health plan of care
* Identify discharge planning needs and implement prior to patient discharge
** Communication:
*** Prepare clinical notes and update the primary physician when necessary
* Communicate with the physician regarding the patient's needs and report any changes in the patient's condition
* Coordinate care plan with community health-related persons
** Additional Duties:
*** Participate in quality assessment and performance improvement programs
* Participate in on-call duties as defined by the on-call policy
* Ensure availability of necessary equipment and services
* Instruct, supervise, and evaluate home health aide care provided every two weeks
* Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed

* Job Requirements:
*** Graduated from an approved school of nursing and licensed in the state of practice
* Minimum of two years of experience, with preference for public health or home care nursing
* Maintains a current CPR certification
* Licensed driver with an insured automobile in good working order
* Self-directed and able to work with minimal supervision
* Excellent observation, problem-solving, and communication skills
* Ability to organize and prioritize workload independently
* No management experience required
* Able to perform required nursing skills
