Hybrid working (office-based approximately 2 days per week, dependent on visits)
We are currently recruiting a Personal Adviser to support care experienced young people as they transition from care into independent adulthood. This is a rewarding opportunity to work with young people aged 16-25, helping them build confidence, independence and positive life outcomes.
The RoleYou will undertake the statutory Personal Adviser role, acting as the key professional and case manager for young people leaving care. You will work closely with young people and their support networks to develop independent living skills and ensure a smooth, supported transition into adulthood.
Key Responsibilities- Act as the statutory Personal Adviser for care experienced young people aged 16-21 (up to 25 where applicable)
- Serve as the key professional and case manager for Former Relevant, and where required Relevant or Qualifying, young people
- Support young people to prepare for leaving care and develop independent living skills
- Provide advice, guidance and practical support relating to housing, education, employment, training, health and wellbeing
- Work collaboratively with families, carers, partner agencies and other professionals
- Develop, implement and review plans that support young people’s transition to independence
- Maintain accurate records and case notes in line with statutory requirements
- Undertake visits and flexible working to meet the needs of young people
- Experience working with care experienced young people or within Leaving Care services
- Strong understanding of the statutory Personal Adviser role
- Ability to manage a caseload and work as the key professional
- Excellent communication and relationship‑building skills
- Full UK driving licence, access to own vehicle and business insurance
- Ability to work flexibly across office, community and home settings
- Confident using electronic case management systems
Hybrid working model. Teams are generally office‑based 2 days per week, with the remainder dependent on visits and service needs.
