We are currently recruiting a Social Care Practitioner to join our Community Area Teams and Acute Hospital Team Adult Social. We have full and part time positions available, in the area teams there is a level of hybrid and face to face visits. In the acute hospitals there is a need to be onsite and working as a multi‑disciplinary team with our health colleagues.
The ILC Social Care team works closely with colleagues in the ILC Health (community based) teams and alongside health colleagues within the hospitals. The team aims to provide a discharge, assess and admission avoidance service within Shropshire, operating with a home first and strengths‑based philosophy, the team works to maximise independence and reduce reliance upon paid services where possible.
We are a seven‑day service working 9‑1 currently at the weekends supporting discharges from hospitals and admission avoidance. For working the weekend you get a day off in lieu for Saturday and time and a half on Sunday. We also work bank holidays working 9‑1 at the current time. You get paid double time and a toil day back.
ResponsibilitiesYou will be responsible for assessment and support of adults who have been discharged from the acute or community hospitals to care homes or their own homes, ensuring the service user is at the heart of all we do.
QualificationsEssential:
Good general education to GCSE level or equivalent including English Language (A‑C).
