We are seeking a compassionate, skilled and experienced Nurse Practitioner to join our community team to help deliver outstanding, person-centred care to patients in their own homes and community settings.

This is a key clinical role for a Nurse Practitioner with extensive specialist knowledge in palliative care and a commitment to ongoing clinical involvement and service development within this field. You will have or be working toward a palliative care degree or equivalent Level 3 Health Degree and/or working towards being a prescriber.

With your professional and friendly approach, will provide specialist assessment, advice, and nursing care to patients with complex palliative and end-of-life care needs. You will also establish and maintain supportive relationships with GPs, wider nursing teams and other partners to ensure the highest quality of care.

As a team player and natural leader, you will contribute to multidisciplinary team meetings to review and manage patient treatment plans and will act as a role model, sharing specialist skills and supporting the development of other practitioners.

You will manage referrals efficiently and ensure appropriate deployment of skilled staff to meet patient needs and on a rotational basis you will provide a triage service.

Leading by example you will also play a supportive role in delivering specialist palliative care education, promoting evidence-based practice and contributing to service development.

Main duties of the job

Provide specialist advice, support, and nursing care to patients whilst establishing supportive relationships and presence with GPs and other community teams within an identified geographical patch (delivered within community and outpatient clinic settings).

Demonstrate palliative assessment skills, and will be able to support, illustrate, and disseminate such skills to other practitioners while benefiting patient care.

With support from the Palliative Care Community Consultant, will assess and support patients who are on our virtual ward and are experiencing complex symptoms or a crisis in their illness.

Attend daily/weekly Multidisciplinary meetings to review and manage patient treatment plans.

Work as part of a multidisciplinary team and other care providers, demonstrating autonomy within a geographical area.

Take a supportive role in the provision of specialist palliative care education, promoting evidence-based practice.

To provide a triage service on a rotational basis, receiving incoming calls, taking messages, and responding accordingly.

To manage referrals promptly through robust assessment and prioritisation processes and deploy appropriately experienced and skilled staff to meet the needs of those referred patients.

To ensure that there is efficient and effective utilisation of resources human and material to deliver optimum quality care to their client group.

To work flexibly to meet the needs of the Hospice at Home service.
