Are you a compassionate, detail-driven professional with a passion for making a real difference in patient care? Shropshire Community Health Trust is seeking a highly organised Discharge Support Worker to join our expanding Integrated Front Door, Capacity Hub Team and Rapid care team within the Urgent and Emergency Care division. The position is an ideal opportunity for Nursing Associates, Social Care Practitioners or Allied Health Support Workers, to shape this new service for our patients.

This is a dynamic and rewarding role where you'll work across division and multi-agency and interdisciplinary teams to support seamless patient journeys. The Urgent and Emergency Division works to prevent avoidable hospital admissions and facilitate timely, safe discharges, working in partnership with health, social care, and voluntary sector organisations. You'll play a key role in co-ordinating, identifying and ensuring patients are supported and transferred to the most appropriate discharge and community pathways.

As the service undergoes exciting transformation, you'll be part of a skilled and forward-thinking team helping to shape the future of patient flow and complex discharge planning.

Main duties of the job

The Discharge Support Worker plays a vital role in directing patient flow across the system by supporting timely referrals between Community and Acute providers and Local Authorities. The postholder will manage all associated administrative functions, maintain accurate system and service reports, and work to maximise flow across the Trust.

As a key member of the Urgent and Emergency Care (UEC) Division, the postholder will work closely with Virtual Wards, the Capacity Hub, the Integrated Front Door Team, and Urgent Community Response Teams to support the delivery of a coordinated and responsive service that improves patient transitions and overall system flow.

Core responsibilities include processing referrals in line with agreed criteria, maintaining an up-to-date caseload on RIO, and working alongside the Clinical Support Coordinator to monitor and manage capacity within the division, ensuring smooth and continuous progression through services.

The postholder will also provide coordination across both services, supporting the identification and pull of suitable patients into the Rapid Care Team, as well as managing and coordinating the current caseload. Operating across the UEC division, the role plays a central part in ensuring safe, timely, and effective patient transitions.

The role operates from 8am to 8pm, seven days a week, including weekends and bank holidays on a rota basis.
