Overview

Physical Therapist Assistant, Home Health — Join Center Well Home Health as part of a team that helps patients regain strength, mobility and independence in the comfort of their homes. You will deliver personalized care focused on rehabilitation and functional improvement to improve quality of life.

Responsibilities
  • Administer the physical therapy treatment plan as developed by the Physical Therapist.
  • Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities to patients.
  • Accurately document daily, weekly, or monthly records of clients and submit clinical paperwork on time each week.
  • Attend educational meetings and coordinate department in-services.
  • Notify the physical therapist or Executive Director of any equipment failures, supply shortages, staff or client changes, and other departmental issues.
  • Schedule clients and perform varied office tasks as needed.
  • Apply concepts of infection control and universal precautions in patient care activities.
  • Accept clinical assignments that align with education and competence to care for patients.
  • Promote compliance with fiscal intermediaries and/or other third-party payors through education, coaching, and assistance as necessary.
Qualifications
  • Associate degree in Physical Therapy from an American Physical Therapy Association.
  • Current state license to practice Physical Therapy.
  • Valid driver’s license, auto insurance and reliable transportation.
  • Proof of current CPR certification.
  • Minimum two years of experience as a Physical Therapy Assistant.
  • Comprehensive knowledge of the physical, emotional, social, and biological changes in the geriatric population.
  • Self-directed, enthusiastic, and able to accept constructive feedback.
  • Good verbal and written communication and organizational skills.
  • Professional image/attitude for patients, families, coworkers, and others.
  • Meets mandatory continuing education requirements and maintains clinical competency in Physical Therapy practice and theory.
  • Ability to work a flexible schedule and travel locally.
  • Able to maintain confidentiality and keep current licensure certifications and continuing education requirements.
  • Must read, write, and speak fluent English.
  • Must have good and regular attendance.
Pay and Schedule
  • Base pay range: $58,400.00/yr - $80,000.00/yr
  • Pay range (per visit/unit): $37.00 - $52.00
  • Annual pay varies based on location, skills and experience.
  • Scheduled weekly hours: 1
