Location:

  • Shorewood, IL Pay Range: $35 – $45 per hour

    Schedule:

    Day Shift – 7:00 AM – 7:00 PM Signing Bonus + Benefits Available Required Skills & Experience G-tube Trach The ideal candidate will display compassion, education, proficiency and experience while assisting the patient.
  • This critical role assists in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.
Responsibilities:
  • Observe, assess, and provide nursing diagnoses for patients.
  • Counsel patients and families, deliver health teaching, and focus on health maintenance and illness prevention.
  • Maintain a comprehensive clinical record for each patient under care, ensuring accurate and timely documentation.
  • Provide progress notes to the patient’s physician or podiatrist when the patient’s condition changes, deviates from the plan of care, or every 90 days (home nursing agency).
  • Assign tasks to home health aides, prepare written instructions, and supervise their activities in the home setting.
  • Direct and oversee the activities of LPNs to ensure adherence to the care plan and quality standards.
  • Administer medications and treatments as prescribed by the patient’s physician or podiatrist, following all safety and regulatory guidelines.
  • Act as the coordinator of the health care team to maintain seamless care across the continuum, ensuring effective communication and linkages between all care providers.
Qualifications:
  • Active and unrestricted Registered Nurse (RN) license in the state of Illinois.
  • Graduate of an accredited nursing program.
  • Previous experience in home health care or a similar setting is preferred.
  • Strong clinical assessment and documentation abilities.
  • Excellent communication and coordination skills to work effectively with patients, families, and health care teams.
  • Compassionate approach to patient care with a commitment to health maintenance and illness prevention.
