Location:Shorewood, IL
Pay Range: $35 – $45 per hour
Schedule:Day Shift – 7:00 AM – 7:00 PM
Signing Bonus + Benefits Available

The ideal candidate will display compassion, education, proficiency and experience while assisting the patient. This critical role assists in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.

Responsibilities:

  • Observe, assess, and provide nursing diagnoses for patients.
  • Counsel patients and families, deliver health teaching, and focus on health maintenance and illness prevention.
  • Maintain a comprehensive clinical record for each patient under care, ensuring accurate and timely documentation.
  • Provide progress notes to the patient’s physician or podiatrist when the patient’s condition changes, deviates from the plan of care, or every 90 days (home nursing agency).
  • Assign tasks to home health aides, prepare written instructions, and supervise their activities in the home setting.
  • Direct and oversee the activities of LPNs to ensure adherence to the care plan and quality standards.
  • Administer medications and treatments as prescribed by the patient’s physician or podiatrist, following all safety and regulatory guidelines.
  • Act as the coordinator of the health care team to maintain seamless care across the continuum, ensuring effective communication and linkages between all care providers.

Qualifications:

  • Active and unrestricted Registered Nurse (RN) license in the state of Illinois.
  • Graduate of an accredited nursing program.
  • Previous experience in home health care or a similar setting is preferred.
  • Strong clinical assessment and documentation abilities.
  • Excellent communication and coordination skills to work effectively with patients, families, and health care teams.
  • Compassionate approach to patient care with a commitment to health maintenance and illness prevention.
