Job Overview
Telephonic position providing ongoing monitoring and dosage management of anticoagulation patients and communicating drug therapy plans clearly to the patient or caregiver. Provide necessary education and support to patients and caregivers while performing ongoing therapy management. Monitor patient progress toward established goals to achieve optimal outcomes. Maintain complete, timely and accurate clinical documentation for each patient.

Schedule
: days
- Provide initial and ongoing assessment of patients, including evaluation of drug therapies, medical conditions, diet, and compliance.
- Provide monitoring of appropriate labs for anticoagulation patients.
- Integrate all appropriate current and past information and apply to established protocols for dosing and monitoring plans.
- Communicate effectively with patient, other care facilities, home care, clinic personnel, pharmacies, and providers regarding care.
- Triage telephone calls from patients, caregivers, or providers.
- Graduate of an approved school of professional nursing
- 1 year RN direct patient care experience
- MN Registered Nurse (RN) License
- WI Registered Nurse (RN) License may be required based on clinic assignment/patient coverage
- Bachelor of Science Nursing
- Previous clinic experience with anticoagulation medications
- Telephone clinic triage experience
- Anticoagulation Provider Cert management certification
