Outpatient Physical Therapy (at-Home Care)
A premier orthopedic group is seeking a motivated and skilled Outpatient Physical Therapist to deliver outpatient-level care directly in patients’ homes. This unique position offers the opportunity to collaborate closely with orthopedic surgeons and the hospital’s management company, ensuring a streamlined, high-quality care experience from surgery through recovery.
Responsibilities- Provide evidence-based physical therapy services to post-surgical and orthopedic patients in their homes.
- Collaborate with surgeons, physician assistants, and care managers to ensure continuity of care.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Educate patients and their families about treatment options, exercises, and preventive measures.
- Conduct comprehensive evaluations and implement tailored treatment plans.
- Document all care in alignment with outpatient standards and regulatory requirements.
- Participate in ongoing case reviews and care team meetings.
- Work with and supervise PTAs.
- Current and valid Physical Therapist license in TX.
- Minimum 1 year of outpatient orthopedic or home health experience preferred but not required.
- If new grad, training and mentorship will be provided.
- Strong interpersonal and communication skills.
- Self‑directed, with the ability to work independently in the field.
- Reliable transportation and willingness to travel within service area.
- Excellent communication skills for interacting with patients, families, and interdisciplinary teams.
