RN-Discharge Planning and Special Programs Coordinator
Join to apply for the RN-Discharge Planning and Special Programs Coordinator role at United Surgical Partners International, Inc.
Responsibilities- Perform early and ongoing discharge assessments addressing clinical, psychosocial, and environmental needs.
- Develop individualized discharge plans and collaborate in interdisciplinary rounds.
- Provide patient and caregiver education using evidence‑based frameworks and teach‑back methods.
- Manage and run UM Committee.
- Document all discharge planning activities in compliance with CMS and accreditation standards.
- Direct screen colonoscopy program coordination.
Skills and Qualifications
- Graduate of an accredited RN School of Nursing.
- Experience 2+ years in acute care; preferred experience in case management, GI, wellness programs.
- Current RN license in the state.
- BLS certification.
- Current ACLS/PALS preferred or included in education plan.
- Ability to make decisions based on nursing judgment.
- Able to prioritize and organize workload to meet the needs of patients and unit.
- Strong communication skills, verbal and written.
