Position: Day Break Supervisor

Day Break Supervisor

The Hub on Smith – Sheridan, WY

Salary: $30,000–$40,000 per year

Overview

Lead our adult day program, providing support and enrichment for older adults and their families. Manage daily operations, guide a caring team, and maintain a safe, engaging environment rooted in dignity and respect.

Key Responsibilities
  • Oversee program operations in compliance with State of Wyoming Adult Day Program standards.
  • Develop and implement client‑centered programs and service plans.
  • Assess client progress and facilitate discharge planning.
  • Supervise and develop staff and volunteers, conduct annual evaluations, and set team goals.
  • Schedule activities, maintain documentation, and report monthly service units, billing, and state‑required reports.
  • Manage program budget, participate in annual planning and fundraising.
  • Represent the organization both internally and externally, collaborate with families, staff, and community partners.
  • Ensure rapid response to health and safety concerns and emergencies.
Qualifications
  • Minimum 2 years experience with older adults or individuals with disabilities in a day program, healthcare, or community services setting.
  • Post‑secondary education: 48 semester hours or 72 credit hours preferred.
  • Ability to obtain a CNA license within 9 months of hire.
  • Strong leadership, communication, and problem‑solving skills.
  • Proficiency in state program regulations and adult day program standards.
  • Valid driver’s license and reliable personal vehicle for occasional travel.
Physical Demands
  • Ability to lift 50 pounds occasionally and perform frequent standing, walking, bending, and reaching.
  • Assist participants with mobility, including transfers.
  • Respond quickly in emergencies and evacuations.
  • Prolonged periods of sitting and computer use for documentation.
  • Exposure to bodily fluids and cleaning agents possible.
Compensation

Annual salary range: $30,000–$40,000. Additional benefits may apply.
