Responsibilities- Conduct Medicare Annual Wellness Visits in accordance with CMS guidelines under provider supervision
- Collect and document comprehensive patient histories, including medical, surgical, family, and social history
- Record allergies and immunization status
- Perform and document required screenings: depression (PHQ-2/PHQ-9), cognitive impairment, fall risk, tobacco, alcohol, and substance use
- Record vital signs, height, weight, BMI, and other relevant clinical data
- Identify preventive care gaps and assist with development of personalized prevention plans
- Provide patient education on preventive services, chronic disease management, and lifestyle modifications
- Ensure accurate and timely documentation in the electronic health record (EHR)
- Assist providers with outpatient clinical workflows as needed
- Prepare patients for provider visits and procedures
- Administer injections, immunizations, and medications per scope of practice and provider orders
- Perform basic clinical procedures consistent with LPN licensure
- Coordinate referrals, diagnostic testing, and follow-up appointments
- Communicate test results and care instructions to patients per protocol
- Maintain exam room cleanliness and stock supplies
- Adhere to infection control, safety, and HIPAA regulations
- Support quality initiatives, population health programs, and preventive care metrics
- Collaborate with providers, care coordinators, and administrative staff
- Demonstrate compassionate, patient-centered care and professionalism
- Floating to other GFP offices may be required
- Current, unrestricted Licensed Practical Nurse (LPN) license (state-specific)
- Graduation from an accredited practical nursing program
- Knowledge of outpatient clinical workflows
- Basic computer skills and experience with EHR systems
- Strong communication, organizational, and documentation skills
- Experience conducting Medicare Annual Wellness Visits
- Familiarity with CMS preventive care guidelines
- Outpatient primary care or internal medicine experience
