Nurse Care Manager

• Build and maintain relationships with local providers and referral sources.
• Lead community outreach to increase client census.
• Provide clinical leadership for the care team.
• Conduct initial assessments and weekly care management.
• Triage clinical situations and coordinate care with the interdisciplinary team.
• Create care plans for caregivers.
• Provide clinical support to in‑home caregivers.
• Deliver nursing education to patients, families, and staff as needed.
• Perform basic bedside nursing care.
• Conduct intake assessments and consultations.
• Conduct community outreach and support.
• Manage medication and care coordination.
• Advocate for patients.
• Transport clients to appointments and outings.

• Associate Degree in Nursing (minimum) or Bachelor’s/Master’s Degree in Nursing (preferred).
• Registered Nurse (RN) license in the state of application.
• Minimum of two (2) years’ experience in geriatrics, home care, home health, and/or care management.
• Preferred experience in Med/Surg, Emergency, or Intensive Care.
• Ability to work in a team environment.
• Effective professional communication with clients, families, physicians, attorneys, and other professionals.
• Excellent written and verbal communication skills.
• Technical literacy.
• Valid driver’s license and proof of current auto insurance.
