Atrium Health Cleveland Pines is an award‑winning nursing and rehabilitation facility with 120 beds offering short‑term rehab, long‑term care, palliative care, and hospice care. The LPN role includes assessments, medication administration, bedside tasks, and documentation. Leadership encourages growth and development. EssentialFunctions
- Assessment involves determining nursing care needs based on data about patient/resident health status.
- Recognize and determine need for immediate intervention.
- Document patient/resident observations, treatments, response to treatment, and nursing care using the nursing process; assist in treatments, procedures, and discharge planning.
- Assist RN with plan‑of‑care development/revision by providing data and identifying potential goals and interventions for review.
- Communicate significant changes in patient/resident status to appropriate clinical staff.
- Implement the plan under the supervision of RN, physician, or other licensed practitioner, ensuring tasks are completed.
- Monitor patient/resident progress and goals, document in medical records, and report to RN.
- Organize workload and prioritize responsibilities to deliver quality care efficiently.
- Perform only nursing skills within legal boundaries and facility policies; attain current competence.
- Document assessments, procedures, patient/resident/family education, and communication with the healthcare team in a timely manner.
- Provide and assess effectiveness of patient/resident and family education as directed by RN within scope of practice.
Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must lift and support up to 35 pounds. Must speak English clearly. Requires intact senses and finger dexterity. Must respond quickly to changes. Physical abilities testing required. Additional department‑specific requirements may apply.
Education, Experience, and CertificationsGraduate of an accredited School of Practical Nursing. BLS required. Current LPN license or temporary license as a Licensed Practical Nurse Petitioner in the state of residence or work location. For National License Compact states, meet licensure requirements in home state; otherwise current LPN license or temporary license required in the state where the LPN works. Additional education, training, certifications, or experience may be required by the nurse leader.
