- Undertake strengths‑based assessments in line with the Care Act 2014, ensuring that service users and carers are involved in care planning.
- Develop person‑centred support plans to promote wellbeing and independence, using a creative approach to reduce reliance on long‑term services.
- Complete safeguarding enquiries under Section 42 of the Care Act, following local safeguarding protocols.
- Provide social work input to multidisciplinary team discussions, contributing to reviews, hospital discharges, and community‑based care coordination.
- Work closely with health professionals, housing teams, voluntary services, and other partners to ensure joined‑up care and support.
- Ensure timely recording of assessments, reviews, decisions, and interventions using case management systems.
- Contribute to the prevention of hospital admissions and timely discharge planning.
- Apply relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, Human Rights Act, and Deprivation of Liberty Safeguards (DoLS) in all casework.
