Job overview

Combined Community and Acute Care group are looking to recruit motivated and enthusiastic Nurse Advisors to work on part time or full time basis.

This role is varied ensuring robust procedures are in place to deliver high standards of clinical care and safety. It involves supporting the delivery of city‑wide projects, offering timely intervention to support recovery and independence.

The post will involve working with a range of colleagues in primary and secondary health, social care and the voluntary sector. Applicants must be a Registered Nurse with post‑registration experience in primary care and specialist long‑term condition management.

Responsibilities
  • Deliver high‑quality nursing care through telephone triage assessment and signposting to appropriate health care providers.
  • Assess health care needs and ensure referral to the appropriate provider in accordance with clinical need, including social and voluntary sector signposting.
  • Work autonomously to provide a holistic approach to patient support, education and advice across the interface pathway.
  • Take responsibility for the service during defined duty periods, including supervising and leading staff in single point of access when absent.
  • Maintain PROUD values and behaviours within service delivery.
Qualifications and experience
  • Registered Nurse with current registration.
  • Post‑graduate level education (academic level
    6) or demonstrable knowledge, training and experience.
  • Line management experience and knowledge of Human Resources processes.
  • Specialist training experience, diploma level courses.
  • Telephone triage experience.
  • Knowledge of specialist long‑term condition management and acute episode management.
  • Proficiency in IT and Systm One is desirable.
Working pattern

Includes weekends, bank holidays and unsociable hours. This vacancy is advertised on a fixed‑term contract basis for a period of a maximum of twelve months due to maternity leave of the substantive post holder.
