The Health Inclusion Team is a unique team offering holistic assessment and intervention to vulnerable families in Sheffield. An opportunity to join the team as a Specialist Health Visitor, with agile working, has become available to cover a secondment.

A Specialist Health Visitor will support the team by providing innovative nursing and health visiting services to asylum seekers, Gypsies and Travellers and people who live in homeless accommodation, women's refuges and interim supported accommodation.

As a Specialist Health Visitor, you will undertake family health assessments for the whole family and carry out child health surveillance in accordance with the Healthy Child Programme.

You will have the opportunity to make a difference to the lives of families and individuals by working in partnership with service users and other agencies to provide effective healthcare, preventing ill health, promoting positive health and reduce health inequalities.

The post holder will need to be a Specialist Community Public Health Nurse with current NMC registration.

The team works closely with a range of agencies to ensure a strong and coordinated service, working within the NMC Code of Professional Standards and Clinical Governance Frameworks.

The Specialist Health Visitor will engage families in their own homes and community settings with the aim of undertaking a comprehensive assessment of each family member that takes account of their health and social needs. They will consider a family's vulnerabilities and historical trauma before agreeing a plan of intervention that actively supports the family to access services and proactively prevent adverse health outcomes.

The post holder will be confident in prioritising and promoting the welfare of children of all ages, working to Safeguarding policies and procedures and contributing to multiagency working.

A large part of the role includes working with voluntary and statutory agencies, requiring excellent communication skills and commitment to Sheffield Health Partnership's University Foundation Trust values.

Principal duties
To identify, prioritise, develop and implement programmes of care to meet individual and local needs, considering cultural diversity and ensuring prioritisation of workload as necessary

To work with families in a variety of settings including home visits and clinic, providing evidence-based advice and support.

To undertake family health assessments and child health surveillance in accordance with the Healthy Child Programme.

To provide relevant information to families, to assist informed choices about their own and their familys health.

To operate within Sheffield Safeguarding Children Board and SHSCFT Child Protection and Safeguarding Procedures. To maintain and update knowledge around safeguarding children and young people and ensure that practice complies with local and national policies and legislation. To work closely with the Sheffield Safeguarding Advisory Service and other key agencies to ensure the highest level of protection for children  attend safeguarding or child in need meetings and provide reports as necessary.

• oUnderstanding and skills in carrying out health needs assessment for adults, children, and young people 5-19 years of age.
• oAwareness of national and local policies around the Healthy Child Programme and the Public Health agenda
• oA sound knowledge of local and national adult and child protection and safeguarding guidelines and policies.
• oUnderstanding of the issues relating to vulnerability and the health and social needs of homeless, Gypsies and Travellers, asylum seekers and refugees.
• oUnderstanding of Trauma informed practice.
