Birch Avenue Nursing Home provides 40 nursing home beds for people with complex dementia. The Registered Provider and owner is South Yorkshire Housing Association, and we work in partnership with Sheffield Health Partnership University. The focus of the home is improvement and empowerment for the service user to engage in quality life experience.
We are seeking, caring, motivated and enthusiastic Healthcare Support Workers to join our Older Adults Specialist Care Team, supporting individuals living with dementia within a multi‑disciplinary environment.
Successful candidates will demonstrate empathy, reliability and commitment to maintaining dignity and respect for the people we care for.
Previous experience working with those diagnosed with dementia is a desirable part to play, however, not essential as successful candidates will be given induction, training, and opportunities for personal and professional development, including individual supervision, Personal/ Professional Development Review and opportunities offered for career development.
As part of the team, you would be expected to be able to work shifts over a 24‑hour basis including weekends and Bank Holidays, so a flexible approach is key.
Basic computer skills, verbal and written communication are essential to navigate this frontline care role.
Main duties of the jobThe main duties of the role include:
- To implement identified care plans-direct delivery of care to our residents, under direction/supervision of Nurse in charge as part of their daily care needs
- Report observations of residents to Nurse in charge, to ensure appropriate review and evaluation of care plans take place. Monitor and report changes in patients' physical and emotional well‑being to the Registered Nurse in Charge/ nursing team in order to appropriately elevate concerns, all working towards working together as a multi‑disciplinary team.
- To maintain and promote person centred care in all aspects of service delivery ensuring resident/relative engagement and choice is part of this. Providing high quality, person‑centred care, to those with complex dementia, older adult physical health and mental health conditions.
- To promote independence, choice, dignity, and respect throughout all hands‑on care delivery. Supporting patients with personal care, mobility and activities of daily living by assisting with personal cares and activities of daily living, depending on the level of need assessed in the moment and what is documented within individualised care plans and risk assessments
- As part of the team, you will be responsible for maintaining the highest standards of care, following individual care plans. Maintain accurate and timely records of care in line with Trust Policy and Procedure
