What you will do:

• Conduct comprehensive mental health assessments and deliver interventions as part of the Home Treatment Team’s Gatekeeping service.
• Co-ordinate care plans and implement appropriate support tailored to individual needs.
• Work collaboratively with multidisciplinary teams to ensure patient‑centred care.
• Monitor patient progress and adapt care plans as necessary to optimise outcomes.
• Provide emotional support and guidance to both patients and their families.

Requirements of the Registered Mental Health Nurse:

• A degree or equivalent in Mental Health Nursing is essential.
• Demonstrable experience as a Registered Mental Health Nurse is required to succeed in this role.
• Valid NMC Registration is a must to practice as a nurse in this position.
