An exciting opportunity has arisen to join our multi-disciplinary PCN team.
We are a dynamic, forward thinking PCNteam who are passionate about developing and delivering excellent quality local services to meet the needs of our patients.
The PCN has been successful in becomingpart of a national pilot scheme which will look at ways to make General Practice Sustainable.
We are therefore looking to expand ourteam by employing Salaried General Practitioners to work alongside our practices and PCN staff.
Our PCN staff currently includes Clinical Pharmacists, Physicians Associates, Social Prescribers, Paramedics, Physiotherapists,Care Coordinators, Nurse Associates and Pharmacist Technicians.
Main duties of the jobTo perform as an autonomousgeneral practitioner working independently and with other health care professionals to assess, diagnose and treat the conditions of patients attending within a primary care setting. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. To be involved in the provision of education and training to GP Registrars and other staff. To beprofessionally accountable and responsible for all elements of clinical practice in accordance with the GMC code of conduct ensuring the safety of patients and quality of their care is primary.
The post holder will need to be capable of independent travel across the Rother Valley South area and work flexibly tomeet the needs of the Business.
About usRother Valley South PCN is made up of four GP Practices witha weighted list size of 57,000. Based in South Yorkshire ICB the Network coversa large suburban area of Rotherham.
The PCN formed in 2019 with no changes to its originalmembers to date. The PCN incorporated as an LTD Company in 2023 to further support development within the network.

Clinicalresponsibilities
In accordance with the practice rota, asagreed, the post-holder will make him/her-self available to undertake a varietyof duties including surgery consultations, telephone consultations and queries,visiting patients at home, checking and signing/authorising repeat prescriptions and dealing with queries, paperwork and correspondence in atimely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
In consultation with patients and in line with current practice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and fromthe caseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Compiling and issuing computer-generated acuteand repeat prescriptions
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
The post-holder will be required to visit patients during surgery time in an emergency if clinically indicated.
The post-holder will undertake a fair % of OnCall sessions per week as allocated on the practice rota and participate in theextended hours enhanced service.

Person Specification Experience- Experience of working in a primary care environment
- Experience of continued professional development
- Experience of QOF and clinical audit
- General understanding of the primary care contract
- Minimum of two years as a salaried GP
- Experience of medicines management
- Experience of ICB initiatives
- Full GMC registration
- National Performers List registration (England), noting that confirmation on this list is adequate for DBS confirmation
- Appropriate defence indemnity (MPS/MDU)
- Evidence of current validation
- Evidence of last appraisal
- Eligibility to practice in the UK independently
- Qualified GP
- MRCGP
- Vocational Training Certificate or equivalent JCPTGP
- General Practitioner (Certificate of Completion of Training CCT)
- Excellent communication skills (written and oral)
- Clear, polite telephone manner
- Competent in the use of Office and Outlook
- Effective time management (planning and organising)
- Ability to work as a team member and autonomously
