• Active Registered Nurse (RN) license in the state of Wisconsin — required for Home Health / Homecare RN roles
• At least 1 year of home health or community-based homecare experience within the past 3 years
• Proficient with HCHB (Homecare Homebase) and OASIS documentation — experience with home health documentation systems
• Valid driver's license and reliable transportation for travel within the service area
• Strong communication, organizational, and clinical assessment skills for home health nursing

• Provide skilled nursing care to patients in their homes across Sheboygan County and northern Ozaukee County
• Perform comprehensive patient assessments and develop individualized homecare treatment plans
• Complete accurate documentation using HCHB and OASIS to support home health outcomes
• Collaborate with interdisciplinary team members (case managers, therapists, physicians) to ensure coordinated care
• Meet productivity expectations (minimum 30 points per week) for a travel RN home health assignment
• Provide patient and family education on disease management, medications, wound care, and self-care techniques

• Estimated Weekly Salary: $1850-$1940/week
