• Provides communication between all health care workers, residents, family and community both orally and in written format.
• Assesses the resident’s condition on an ongoing basis utilizing the nursing process. Documents information gathered in accordance with the nursing process and facility policy and procedures.
• Evaluated resident’s condition and identifies the need to notify the attending physician of any change in the resident’s condition, etc.
• Administers medication and treatment and provides direct resident care.
• Participates in the care planning process in coordinating nursing services with other discipline services.
• Directly responsible for supervision of the daily work assignments and performances of assigned activities of nursing staff (i.e. LPN’s, MA’s, C.N.A’s) in accordance with the Nurse Practice Act.
• Prepares and implements employee performance evaluations for LPN’s, MA’s, C.N.A.’s.
• May assume charge nurse duties to meet facility needs.
• Participates in the Quality Assurance Program.
• May perform other varied duties/responsibilities as assigned per facility needs.

• Graduate of Accredited/Licensed School of Nursing.
• Licensed to practice as a Registered Nurse in the State of Wisconsin as defined by S. 441.11(1) Wisconsin Statutes.
• Previous work experience in geriatrics, medical surgical, psychiatric or the Developmentally disabled preferred.
• Understands and effectively carries out oral and written instructions.
• Understands the concept of comprehensive nursing care.
• Ability to plan and direct the work of others.
