• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Must have at least 12 months of Hospice or Home Health experience within the past 3 years

• Coverage Area:
  Sheboygan, Calumet, Southern Manitowoc counties, with the possibility of Northern Washington and Northeast Ozaukee counties as back-up
