Advanced Practice Provider-Hospitalist- St. Nicholas - Sheboygan, WI

Job Summary

Provide assistance to the Hospitalist service in gathering and validating daily patient data, such as vital signs, ancillary reports and results, intake and output reviews, and other clinical information. Perform patient rounds with the Hospitalist and conduct independent rounds when appropriate, identifying patients for discharge. Ensure timely execution of Hospitalist orders and offer input to patient care decisions and planning. Act as a liaison between physicians and case‑management staff regarding discharge planning.

Assist in facilitating timely discharges, including transcribing orders, preparing prescriptions, and confirming discharge plans with family members and case management.

Maintain patient education regarding disease processes and treatment plans. Utilize evidence‑based medicine to improve care and outcomes for Hospitalist patients Work full time, covering vacations, weekends, and holidays.

Coordinate patient admission and discharge planning needs.
• Assist with development and review of policies and procedures.
• Serve as a resource for staff in applying policies and procedures.
• Advocate for patients and significant others.
• Prioritize patient rounds to ensure timely discharge.
• Make phone contacts to primary physicians to confirm dictations and pending labs.
• Follow up with patients within 24 hours of discharge.
• Assist agencies with dispositions, including death certificates, insurance reviews, and billing concerns.

• Willingness to learn new roles within scope of practice.
• Ability to develop and maintain effective working relationships.
• Organizational skills for timely appointments and follow‑up.
• Courteous, service‑oriented interaction with patients, families, and colleagues.
• Participation in NP/PA activities within the community.
• Experience in a hospital setting preferred.

Advanced Nurse Practitioner – Registered nurse in Wisconsin, graduate of a master’s level nurse practitioner program, certified by ANCC or ANA, and current CPR and ACLS certification preferred.

Physician Assistant – Graduate of an accredited PA program, licensed by Wisconsin, certified by the National Commission on Certification of Physician Assistants, and current CPR and ACLS certification preferred.
