Overview
Part‑time 1st shift opportunity in Sheboygan providing community‑based support for individuals managing mental health and/or substance use disorders. Support individuals in remaining independent in the community by providing supportive skill development and medication monitoring 1:1 in participants homes and in the community. Starting salary range $17‑18. Training and CCS certification provided upon hire. Program operates 365 days a year with opportunity for more hours.
Responsibilities- Provide supportive skill development in participants homes and in the community.
- Perform medication monitoring 1:1 with participants.
- Assist individuals in remaining independent in the community.
- High school diploma.
- Ability to do the following tasks: alternating between sitting and standing, walking, stairs, stooping, lifting 10 lbs, reaching overhead and below.
- Experience in adult care, mental health or substance use services preferred.
- Caring and person‑centered approach.
- Valid Driver's License and eligibility to be covered by Elevate insurance coverage required to drive Elevate vehicles.
