Home Health RN Case Manager

Overview

Loving Care In-Home Health Services is a family-owned home health, hospice and private duty services provider since 1997. Our health care services are available to people in 16 counties within the central Oklahoma area.

We are currently accepting applications for a full-time RN Home Health Case Manager for our Shawnee Home Health office. This position will be responsible for delivery and coordination of care to assigned home health patients. This area will include Shawnee, Tecumseh, Prague, Choctaw, Seminole, Wewoka and surrounding areas.

• Deliver and coordinate care to assigned home health patients in the Shawnee area and surrounding towns.
• Collaborate with the established home health care team to plan and implement patient care.
• Communicate effectively with patients and family members; ensure patient needs are addressed in a timely manner.

• Active State of Oklahoma RN license without restrictions.
• Current CPR certification.
• Current valid Driver's License.
• Current required Automobile Insurance requirements.

• Must be able to manage and coordinate all aspects of patient care to assigned home health patients.
• Must be able to work with the established home health care team.
• Must have ability to communicate and interact with patients and family members.
• Must possess good computer skills.
• Must be able to work independently and organize/prioritize schedule.
