The speech-language pathologist provides high-quality patient care through evaluation, treatment planning, therapeutic intervention, family education, and thorough documentation. This role advocates for patients, supports interdisciplinary collaboration, and participates in quality assurance and professional development initiatives. The SLP works closely with the Facility Rehab Director to ensure excellent clinical outcomes and compliance with organizational, state, and federal standards.
Job Responsibilities Clinical CareConduct evaluations according to physician orders, payer guidelines, and facility protocols. Develop individualized treatment plans that align with benefit levels and support optimal functional outcomes. Provide direct treatment and therapeutic interventions to patients. Screen individuals to determine need and eligibility for therapy services. Ensure patient and staff safety at all times through sound clinical judgment.
Communication & CollaborationCommunicate patient status, progress, and needs to patients, families, caregivers, and care team members. Collaborate with interdisciplinary teams for coordinated care, co-treatment opportunities, and protocol development. Provide education to staff, third-party payers, and healthcare professionals regarding the benefits and outcomes of interventions. Make appropriate referrals to other therapy disciplines or care providers.
Documentation & Administrative DutiesComplete evaluations, daily notes, summaries, and reports accurately and on time. Utilize EMR and rehabilitation software systems to enter and maintain patient information. Submit billing, payroll, and expense documentation promptly. Maintain accurate physical and electronic records in compliance with regulations.
Qualifications- Graduate of an accredited Speech-Language Pathology program.
- Active, unencumbered state license as a Speech-Language Pathologist.
- CPR certification (as required for license renewal).
