Job Summary
RN plays a crucial role in coordinating and overseeing the care of members receiving health services in their homes. This professional is responsible for assessing members' needs, implementing individualized care plans, and ensuring that all necessary services are delivered effectively. They collaborate with healthcare providers, members, and families to facilitate communication and ensure continuity of care.
Must be willing to travel to Seminole, Pottawatomie, Pontotoc, and Cleveland counties.
- Sign‑on bonus available based on experience.
- Education:
Technical/Associates degree. - Licenses/Certifications:
RN, Current CPR certification. - Experience:
RN with one year paid professional experience preferred.
- Provide accurate, timely, complete, and acceptable documentation.
- Be alert to changes in the level of care and initiate necessary procedures to assure members continued safety.
- Provide direct nursing care.
- Educate patients and families.
- Inform supervisor immediately of changes in member condition.
- Assist with the transition of the individual from services, if necessary, and planning for any future needs.
- Conduct comprehensive assessments to understand the individual's functional and cognitive capacity, needs, strengths, abilities, and available resources.
- Initiate appropriate preventative and rehabilitative needs.
- Contact and coordinate with both formal and informal providers to arrange for the services outlined in the service plan.
- Attend meetings involving the agency when requested.
- Visit member in accordance with Medicare, Advantage, or other insurance and company guidelines.
- All other duties and responsibilities assigned by management.
- Effectively communicate with individuals, families, and other healthcare providers.
- Familiar with available community resources and how to access them.
- Organizational, time‑management, and problem‑solving skills.
- Knowledge of healthcare regulations.
- Interpersonal skills.
- Effectively document information and close attention to detail.
