Overview

A Case Manager plays a crucial role in coordinating and overseeing the care of members receiving health services in their homes. This professional is responsible for assessing members' needs, developing individualized care plans, and ensuring that all necessary services are delivered effectively. They collaborate with healthcare providers, members, and families to facilitate communication and ensure continuity of care. Sign-on bonus available based on experience.

Must be able to travel to cover the following locations:
Sulphur, Davis, Wayne, Ardmore areas; the counties include Love, Murray, Garvin, Carter and McClain.

Qualifications

Education:

Bachelor’s degree and/or one paid year of professional experience with aging or disabled population, performing duties which encompass the core functions of case management including; assessment, planning, implementation, monitoring and reassessment.

  • Advantage Case Management Certification preferred
  • Licenses/Certifications:

    RN/LPN
  • Experience:

    RN/LPN with one year paid professional experience preferred.
Essential Functions
  • Identify member needs, goals and abilities
  • Ensure ongoing coordination
  • Prepare and submit a comprehensive, written service plan for each member
  • Implement certified Advantage service plan
  • Monitor delivery of services
  • Identify high risk situations
  • Orient and educate members
  • Modify services to meet needs of members
  • Ensure confidentiality according to all relevant laws and regulations
  • Maintain adequate records
  • Justifies expenditures and monitors and contains cost of plan
  • Assist in implantation of inservice education for field employees
  • All other duties and responsibilities assigned by management
Knowledge/Skills/Abilities/Expectations
  • Excellent verbal, interpersonal and written communication skills
  • Excellent problem-solving skills
  • Ability to use a computer and relevant data bases, especially for required reporting
  • Ability to organize and manage a caseload in accordance within agency standards and in compliance with policies and procedures of contracted programs
