This is an exciting opportunity for an enthusiastic, motivated Band 5 Community Nurse to join a dynamic and developing patient‑focused rehabilitation service, based in Selby and supporting people in their own homes.
Prior understanding of Community nursing is desirable. Further competency training and development is expected and will be supported. You will be required to work closely with the Out of Hospitals Directorate team and undertake a wide variety of tasks.
You will have excellent communication and organisational skills, work flexibly, and be a strong team player. You will be providing support to students and junior/non‑registered members of staff.
Responsibilities- Provide holistic assessments as well as specialist nursing treatment and management of patients, providing rehabilitation to patients as part of a Multi‑Disciplinary Team, within the Selby locality.
- Work as part of an established team, helping individuals stay independent by receiving rehabilitation and care in their own home rather than hospital or care facilities.
- Represent the service in a seven‑day service between the hours of 8 a.m. – 8 p.m.; cover all hours on a rota with flexible and varied shift patterns.
- Work closely with all partners, including hospital wards, GP’s, A&E, Social Services and the voluntary sector, supporting hospital discharges and preventing unnecessary admissions.
- Transport yourself and equipment across the locality during the course of a normal working day.
Band 5 Community Nurse with prior experience or understanding of Community nursing, strong communication and organisational skills, ability to work flexibly and in a team. Knowledge of Multi‑Disciplinary Team working and rehabilitation principles is desirable.
Interview Process- Interviews for this position will be held in person ONLY at one of our Trust sites. Remote interviews will not be available.
