The Lead Support Professional (LSP) in Community Living supports the CVA and CHS missions by providing supervision of the day to day activities of Individuals with intellectual and developmental disabilities.

• Aid Individuals by providing physical and emotional support; and maintaining a safe, clean and comfortable living environment through general housekeeping duties (which may include making beds, cooking meal, laundry, care of equipment, etc.)
• Empower Individuals by seeking opportunities where Individuals may demonstrate independence; aiding and assisting with skill development in areas of self-help, self-determination, and self-advocacy; and observing, encouraging and recording Individuals’ progress toward established goals
• Ensure regulatory compliance by managing the health and regulatory documentation for the team and the Individual(s) served; ensuring required documentation is completed to standard and in a timely fashion within established guidelines; actively facilitating and participating in trainings and meetings; communicating and collaborating with Professional Managers for the health and welfare of the program and the Individual(s) served; participating in a tiered on-call system which includes working outside of regularly scheduled hours to ensure adequate coverage;

• Integrate Individuals into Community by encouraging and facilitating community connections; assisting in building meaningful relationships with others in their community; and encouraging participation in activities and programs which provide opportunities to spend time constructively and enjoyably
• Lead a team of Direct Support Professionals by supervising and training team members; scheduling and supporting team members; administering performance evaluations, and directing performance improvement; reviewing timesheets, and managing time-off requests of team members per policies and procedures, communicating and collaborating with Professional Managers for managing program vacancies; communicating professionally with courtesy and kindness with team members; being a role model for the team, and setting the standard of excellence for their team
• Strengthen positive work culture by participating in effective, courteous and professional communication; showing kindness and patience to Individuals and co-workers; demonstrating respect and responsibility in daily work; and practicing advocacy and encouragement in all aspects of employment
• Support the Health of Individuals by communicating with healthcare providers and pharmacies, professional therapy services, and other external agencies for the benefit of the assisting them with personal care and hygiene activities; assisting them with administering medications; and assisting with therapies or other medically related needs.
