Full-Time RN Clinical Manager – Hospital at Home Program at Accord Care

Location:

Stuart to Vero Beach, FL (Hybrid)

Schedule:

Monday–Friday, 8:00 AM – 5:00 PM

Salary: $72,000 annually + Benefits

• Lead and mentor home health staff and aides
• Oversee patient care coordination and program operations
• Ensure clinical excellence and compliance with state and federal standards
• Collaborate with referral partners, physicians, and interdisciplinary teams
• Enjoy a consistent Monday–Friday schedule and supportive team environment

• Manage patient intake, authorizations, and care planning for skilled and non-skilled services
• Conduct start-of-care and supervisory visits, as well as orientations and training for staff
• Provide clinical oversight aligned with best practices and agency policies
  
  Support and mentor field clinicians to ensure quality care delivery
• Maintain compliance with all regulatory and accreditation requirements

• Active RN license in Florida (required)
• Minimum 1 year of home care experience (preferred)
• Experience with Hospital at Home programs (highly preferred)
• Valid driver’s license and reliable transportation
• Strong communication, leadership, and time management skills
• EMR proficiency;
  Alaya Care experience a plus
