Location: North Seattle / Aurora
Salary: $73,500 - $75,500

- Title: Outreach Care Coordinator
- Location: North Seattle / Aurora region; based in Seattle City Council District 5; works with North Outreach Team and coordinates with City of Seattle UCT for shelter referrals at eligible sites and partners in neighborhoods such as Lake City and Aurora Ave.
- Schedule: Full-time onsite;
Monday–Friday, 8:00am–4:30pm - Summary: The Outreach Specialist engages clients living on streets and encampments, provides low-barrier resource navigation, maintains engagement with case management, and builds partnerships to coordinate care.
- Help ETS carry out its mission by collaborating with staff to transform systems of harm and inequity and improve community health and safety related to substance use and homelessness.
- Support organization-wide initiatives to reimagine interconnectedness within the community and address aspects of society shaped by white supremacy and settler colonialism.
- Provide outreach to people living outside in Seattle D5 to conduct health and needs assessments and recommend services.
- Visit sites where individuals experiencing homelessness are located to engage potential clients and explain available approaches.
- Assess needs for mental health, primary medical care, and substance use according to stage of change; focus on reducing harmful consequences.
- Assess the scope of need among the unhoused population in D5 and collaborate with government and community partners to advocate for access to or creation of adequate services.
- Refer clients to resources and support ongoing engagement with services.
- Develop and maintain positive working relationships with program partners and referral sources to facilitate interagency collaboration and avoid service duplication.
- Participate in case conferencing meetings with outreach and other homeless service providers to discuss high-barrier clients.
- Maintain up-to-date data per REACH Outreach standards to coordinate care and have accurate site information.
- Document activities in accordance with agency, contract, state, and federal requirements.
- Coordinate with government and community entities to provide a coordinated system response, including but not limited to: city/county coordinators, public health assessments, hygiene and sanitation services, waste removal, law enforcement coordination, and benefits assistance.
- Participate in stakeholder, government, and community meetings to help shape an effective system response.
- Maintain professional confidentiality while communicating with partners to protect client privacy and advocate for needs.
- Utilize clinical supervision, psychiatric consultation, and peer support to explore intervention strategies, counter transference, resource development, and self-care.
- Provide services consistent with ETS/REACH policies and procedures.
- Local travel is a job requirement.
- Attend REACH team meetings and other required meetings with program partners.

- Education and/or Lived
Experience:
High school diploma or equivalent required; undergraduate degree with coursework in social services preferred. - Minimum two years of experience in social work or outreach programs.
- Ability to respectfully engage with individuals experiencing homelessness and behavioral health conditions.
- Experience working with complex governmental and community partners to maintain service provision.
- Experience with harm reduction services is highly desirable.
