Summary of Responsibilities

You would be responsible for overseeing and coordinating the care of Aging Wisdom clients and their families. As a Care Manager, you are often the primary advocate for your clients. This position involves:

  • Conducting home and facility visits in the Greater Seattle area
  • Performing biopsychosocial assessments; developing, implementing, and monitoring care plans
  • Collaborating with providers across the healthcare continuum
  • Supporting clients in crisis and during end of life
  • Navigating complex family dynamics and other challenges
  • Participate in community professional networking and business development opportunities
  • Initiate relevant learning and professional growth opportunities
  • Maintaining familiarity and knowledge of local community resources
  • Collaborating with supervisor and larger care management team in regular meetings
  • Providing holiday rotational on-call coverage
  • Adhere to professional code of ethics and standards of practice
  • Be committed to excellence at every level
