Pay Range: $73,500–$75,500 |
Schedule: Monday‑Friday 8:30 am‑5:00 pm

Join Evergreen Treatment Services (ETS) in transforming lives through innovative addiction and social services. Our REACH program works with individuals experiencing homelessness and behavioral health conditions, aiming to reduce drug use, improve housing stability, and promote community health.

Job Summary

  • The Case Manager – LEAD role is part of the REACH team that supports ETS’ mission by providing engagement and intensive case management services to individuals suspected of low‑level drug offenses and/or prostitution.
  • The Case Manager will serve a caseload of approximately 25–30 individuals, offering outreach, long‑term engagement, and supportive services through intensive case management activities.
  • This is a full‑time onsite position, Monday‑Friday 8:30 am‑5:00 pm.

Responsibilities

  • Help ETS succeed in carrying out our mission by collaborating with staff to transform systems of harm and inequity, creating new approaches to improving community health and safety.
  • Assist with the organization‑wide initiative to reimagine our interconnectedness within the community, overcoming aspects shaped by white supremacy and settler colonialism.
  • Provide outreach and intensive case management services for assigned participants.
    • Engage participants on the street and at service provider facilities to establish a working relationship and offer services.
    • Assess participants for severity of chemical dependency and housing status, and determine needs for additional services (medical, mental health, etc.).
    • Assist participants in gaining access to funding programs (e.g., SSI, ABD, VA).
    • Assist participants in finding housing and maintaining occupancy.
    • Develop and implement an individualized Service Plan with participant input, updating it periodically to reflect progress and new needs.
    • Develop and maintain relationships with numerous providers (Sobering Center staff, DSHS workers, treatment providers, shelters, landlords, etc.).
    • Provide structured intensive case management services consistent with program policies.
    • Collaborate with LEAD partners, including Seattle Police Department, King County Sheriff, and prosecutorial offices.
    • Advocate and support participants within the criminal justice system, including court appearances and written communication.
    • Attend operational work group meetings and staff participants with partners.
    • Accompany participants to appointments as needed.
    • Assist participants in developing a spending plan and shopping as needed.
    • Advocate for participants with other service providers, gain entry into service programs, and secure appropriate housing.
    • Identify gaps and barriers in community resources and advocate for systemic changes.
    • Attend REACH and LEAD team meetings and other required meetings.
    • Develop and maintain participant files per program, contract, and state requirements.

Requirements

  • Education: High school diploma or equivalent required. Further education or training is desirable.
  • Experience: Ability to respectfully engage individuals, demonstrate understanding of harm reduction, and have a passion for serving those experiencing homelessness and co‑occurring disorders. Street outreach experience is a plus.
  • Skills in advocacy and support within the criminal justice system, including court appearances and written communication.
  • Strong interpersonal and verbal/written communication skills.
  • Excellent organizational skills with the ability to prioritize workload, work independently, and complete tasks efficiently.
  • Dependable, able to work under pressure, receptive to change, and a cooperative problem‑solving team player.
  • Flexible, with excellent attention to detail and the ability to maintain confidentiality and use discretion with sensitive information.
  • Ability to set boundaries, resolve conflict, and de‑escalate issues.
  • Computer literate with basic knowledge of Microsoft Office Suite or equivalent, including use of email, calendaring, video conferencing, and web searching.
