Position: Full‑time Vietnamese speaking Case Manager I in Aging and Adult Services. Provides culturally competent case management services to elderly persons and adults living with disabilities within the Asian and Native Hawaiian/Pacific Islander community. Services are as holistic, person‑centered, and strengths‑based as possible, with the goal of helping individuals to live a meaningful and independent life in the community.

COMPETITIVE BENEFITS PACKAGE INCLUDES:

  • Pay range: $58,400 to $63,100 annually. Pay can increase based on experience.
  • ACRS pays 100% for employee‑only HMO/Virtual Plus medical benefits. Additional options are covered from 75% to 85%.

ESSENTIAL JOB FUNCTIONS:

  • Work with clients to develop and implement person‑centered, holistic care plans, conduct comprehensive in‑home functional assessments, and collaborate with client/family and community resources.
  • Identify client needs and facilitate access to community resources, manage referrals, and ensure services meet client needs.
  • Authorize services and client choice of qualified providers, travel for home visits, and maintain communication for implementation.
  • Authorize or re‑authorize Medicaid‑funded services via Provider One and inform clients of changes.
  • Advocate for services in citizenship, housing, food assistance, legal, translation, and other areas; manage LTC-related referrals and documentation.
  • Facilitate transitions to nursing homes, assisted living, or adult family homes; coordinate placement and continuity of care.
  • Report as a mandatory APS reporter and document suicidal ideation or behavioral issues.
  • Assist clients/families with home modifications, crisis intervention, and appropriate care setting transfers.
  • Complete documentation on time, participate in QA and process improvements, and meet performance standards.
  • Attend agency, program, and team meetings, consultations, and in‑service training.
