OverviewThis position will provide case management services to families eligible to participate in the YMCA Child Placing Agency Program and Behavior Rehabilitation Services Program. The role includes supporting YMCA licensed caregivers of youth in foster care in the King County, Snohomish County, and Pierce County areas, youth enrolled in intensive in‑home stabilization services, and youth enrolled in therapeutic foster care services.
Case management activities involve coordinating or providing services that enable the child and caregivers to access medical, educational, social, clinical, and other services necessary for the child’s physical, psychological, and developmental health. In addition, you will provide leadership and support to the broader Case Management team and ensure all clients in the CPA and BRS programs receive quality case management services.
This position is located at the Social Impact Center Y, the social services branch of YMCA of Greater Seattle (YGS). The Social Impact Center Y’s mission is to accelerate young people’s ability to build safe, successful, and happy lives. Each year we serve 5,000 children, youth, young adults, and families across King County with programs centered on reducing risk factors, navigating systems for successful life transitions, ensuring stability, safety, and wellness during crises, and building competencies and social capital.
Services include housing, behavioral health, foster care licensing, violence prevention, education, and employment. Many of the young people we serve have experienced foster care, criminal‑justice, and/or homelessness systems.
Monday‑Friday, 9:00 a.m.–5:00 p.m. with flexibility for evenings and weekends as needed to meet program and participant needs.

Hiring Range: $24.00 – $27.00 per hour
Responsibilities- Provide case management and care coordination services, as outlined in the CPA and BRS Contracts, Statements of Work, Handbooks, and Policy/Procedures, to clients enrolled in the CPA and BRS Programs including licensed and unlicensed caregivers of youth in foster care, therapeutic foster care, intensive in‑home stabilization services, and outpatient mental health services.
- Maintain a caseload size in accordance with contractual requirements and guidelines.
- Ensure clients receive all required screenings, assessments, services, interventions, and opportunities needed to fully meet their physical, psychological, social, emotional, relational, developmental, educational, independent‑living, housing, and permanency needs in a timely and effective manner.
- In coordination with the Program Director and the child & family team, ensure clients’ transitions are well coordinated and facilitated, including placements & discharges, and that all required intake, service planning, and discharge/exit documentation is completed fully and on time. Provide leadership and guidance in the placement referral process, and ensure case transfers are smooth and as least disruptive as possible to the client.
- Ensure clients’ parent/child/sibling visitation needs and requirements are met and that natural supports and life‑long connections are developed and stabilized.
- Coordinate and ensure inclusion of the client’s Child/Family Team in the planning and decision‑making process.
- Ensure foster parents/caregivers receive the necessary support services (case‑management, care coordination, respite, resources, etc.) to fully meet the client’s needs.
