RN Home Health Admissions with a $10,000 signing bonus. FULL-time position in the Home Health team for admissions. Service area includes King County. Positions involve admitting patients to service and developing a comprehensive plan of care for patients and their families. Care settings include patients in private residences, adult family homes, and assisted and independent living facilities.

• Comprehensive patient assessment
• Developing the care plan in collaboration with the patient and their family
• Admission tasks including education, coordinating resources, communicating with physicians and other team members, pain and symptom management and skilled physical therapy interventions
• Expand and develop OASIS knowledge/expertise
• Prepare for and participate in admissions process

• Active WA RN license
• OASIS experience
• 1 year experience working in Admissions
• Home Health experience strongly preferred
• Valid driver’s license, auto insurance and dependable vehicle
• Computer skills for charting and documentation
