Summary
King County's Department of Community and Human Services (DCHS) provides equitable opportunities for people to be healthy, happy, and connected to community.
The Health Through Housing (HTH) initiative is King County's long-term commitment to accelerate our region's response to chronic homelessness through the use of repurposed hotels and other existing buildings and funding operations of Permanent Supportive Housing (PSH). Health Through Housing invests in the premise that a person's ability to be housed is a fundamental component of that person's ability to gain and maintain health.
The Paramount Goal of the HTH initiative is the creation and operation of 1,600 units of affordable housing with supportive services for eligible adult households in King County that are experiencing chronic homelessness or that are at risk of experiencing chronic homelessness. In addition to the initiative's Paramount Goal, Health through Housing has seven ordinance-adopted supporting goals, the first of which is to annually reduce racial-ethnic disproportionality among persons experiencing chronic homelessness in King County. HTH is staffed within the Housing and Community Development (HCD) Division of King County's Department of Community and Human Services. The mission of HCD is to increase housing stability and develop strong communities throughout the King County region.

We are seeking a Health through Housing (HTH) Referral & Community Coordinator, Project/Program Manager II, to join an initiative developed to create and operate 1,600 units of affordable, supportive housing for eligible adults experiencing chronic homelessness or at risk in King County. For the first 9 months, you will lead jurisdictional stakeholders and housing operators in administering and continuously improving referral operations to identify and house formerly homeless individuals in a person-centered, transparent way that advances HTH's top goal of reducing racial-ethnic disproportionality among people experiencing chronic homelessness.
After that initial period, you will transition to serving as the primary liaison between PSH sites and the surrounding community, playing a hands-on role in community engagement, implementing good-neighbor processes, and conducting interest-holder mapping, outreach, and mobilization. You will also be a member of the Health through Housing Quality Management Team, led by the Quality Management Coordinator, and supporting the team with compliance audits of HTH housing operators.
This recruitment is to fill a Term-Limited Temporary (TLT) position that is anticipated to be two years in duration but is subject to change dependent on business needs. If filled by a current King County employee who has passed their initial probation, the TLT will be as a Special Duty Assignment (SDA).
Job Duties
To be considered minimally qualified applicant must demonstrate a background that provides the knowledge, skills and ability to:
- Ensure housing operators' compliance with operating procedures and HTH Implementation Plan requirements related to community engagement and local and regional referral. Proactively identify areas for continual process improvement and, when necessary, lead those improvements.
- Independently work to identify potential community-based partners (such as non-profits and human service agencies) in each HTH jurisdiction and build operational partnerships with those interest-holders, sometimes involving formal agreements.
