Job SummaryNeighborhood House has a longstanding commitment to ensure that low-income immigrant and refugee aging and disabled members of our community have the opportunity to age in their community, receiving culturally appropriate services that enable the highest quality of life. The Community Resources Supervisor, who will report to the Community Resources Program Director, has the primary responsibility for successful implementation and oversight of our programs that provide clinical support to older adults, individuals with disabilities, including supervision of direct service staff working with clients from low income, immigrant, and refugee communities.
The Community Resources Supervisor will provide oversight of all day-to-day processes for the program including coordinating referrals processes, supporting case management duties and overseeing the program performance. In addition, they will lead the team in providing data management, and administrative oversight of the program toward the shared goal of providing culturally and linguistically competent services to the program’s clients.
Percentages indicate relative emphasis:
- Supervision (30%)
- Provide supervision of a direct service team of case managers.
- Provide solution-focused oversight and coaching to direct service staff to help meet program goals, problem solve, and ensure effective delivery of services to clients and their families.
- With support from the Community Health Director and Community Resources Manager; regularly observe staff, appraise performance, and provide regular motivational feedback.
- Recruit, hire, train and supervise qualified staff in alignment with Neighborhood House Values and program requirements.
- Train new and ongoing staff on proper use of the various systems and monitor for compliance. Be available to staff to answer questions and troubleshoot problems.
- Coordinate staff trainings based on observations and analysis of staff needs, program leadership and funder requests.
- Program Implementation (30%)
- Monitor and enhance internal and external referral systems to ensure coordinated referrals into the program and among direct service staff to ensure equitable caseloads and manageable expectations for staff.
- Maintain positive relationship with funders and contracted service providers.
- Maintain frequent communication with program leadership to discuss challenges in process or barriers to meeting program goals.
- Participate in internal and external leadership opportunities as assigned; represent NH’s collaborative approach to community support.
- Represent NH by participating in activities or professional networks related to program or other related networks as assigned.
- Compliance (30%)
- Ensure all program regulations, protocols, and client services are in compliance with Neighborhood House and funder guidelines.
- Work closely with program leadership to develop and monitor documentation systems for tracking the multiple deadlines associated with the administrative requirements of the program.
- Regularly review case files for accuracy and completeness.
- Analyze and evaluate program quality and client services, making modifications and adjustments to ensure the best possible client services.
- Direct Service (10%)
- Manage a small caseload of around 10-15 high acuity clients.
- Conduct emergency case management.
- Provide coverage as necessary for case managers.
- Maintain timely and accurate client activities and assist with reporting requirements as necessary.
- Perform other duties as assigned.
- Bachelor's Degree in social work, social sciences, health, human services or related field, or equivalent combination of education, training and experience will be considered.
- Demonstrated ability to lead a team through understanding and adhering to complex Federal and State regulations and completing extensive technical documentation.
