The Community Health Worker is a vital member of the clinic team, dedicated to improving health equity, supporting patients through education and navigation, and fostering strong community partnerships. This role requires compassion, adaptability, and a commitment to continuous improvement and cultural competence.

The CHW acts as a bridge to resources and healthcare, supporting patients in overcoming barriers and navigating complex systems.

  Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
• 1 year Health Related, Community Outreach or Education Experience

• Bachelor's Degree Health Related, Community Outreach or Social Work Related Field Or equivalent educ/experience
• Preference will be given to applicants with lived experience

Pay Range: $22.62 - $35.22 per hour. The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
