About DESCDESC is a nonprofit organization serving people with homelessness, substance use disorders, and serious mental illness. DESC serves almost 3,000 people each day with integrated services and housing.
About the ORCA ProgramThe ORCA Center provides low-barrier, post-overdose subacute stabilization and walk-in MOUD access. The ORCA Patient Outreach Division (POD) delivers field-based MOUD care during business hours across the greater Seattle region.
Job Responsibilities- Provide evidence-based MOUD treatment (buprenorphine LAI, methadone under the 72‑hour rule) in community settings.
- Deliver telemedicine care and remote consultation for field nurses.
- Conduct low-barrier clinical assessments, MOUD initiations, acute follow-up, and ongoing management.
- Provide harm reduction counseling and overdose prevention planning.
- Manage skin and soft tissue infections and connect patients to primary care or mental health services.
- Participate in Seattle Fire Department Mobile Integrated Health joint outreach operations and targeted “hot spot” engagement.
- Support field-based follow-up after overdose, SFD encounters, or ORCA Center visits to ensure MOUD continuity.
- Collaborate with POD nurses and peers for holistic care.
- Coordinate warm handoffs with the ORCA Center, DESC outpatient clinics, community MOUD programs, and hospitals.
- Establish partnerships with SFD MIH, PSH, shelter staff, and community organizations.
- Complete timely documentation that meets DESC, regulatory, and payer requirements.
- Utilize electronic health records effectively.
- Participate in ongoing training on addiction medicine, harm reduction, trauma-informed practice, and equity frameworks.
- Join case reviews, interdisciplinary meetings, and team huddles.
- Engage in reflective practice and support a culture of curiosity, collaboration, and equity.
- Fully licensed as a Physician, Nurse Practitioner (Family or Adult NP), or Physician Assistant in Washington.
- If physician, board certification or ability to obtain within 12 months in EM, FM, or IM.
- Current DEA registration.
- Experience or training in addiction medicine, harm reduction, or OUD care.
- Proficiency with clinical documentation and EHR.
- Desire to serve people with complex needs, homelessness, and serious mental illness.
- Able to work effectively with clients displaying challenging behaviors.
- Adaptable to changing priorities and workflows.
- Able to give and receive constructive feedback.
- Familiar with trauma-informed care, harm reduction, and equity principles.
- Possess cultural humility and ability to work with diverse backgrounds.
- Willing to travel across the greater Seattle area.
- Doctoral degree.
- Experience in low-barrier buprenorphine programs or OTP.
- Background in acute settings (ED, EMS, crisis response).
- Training or experience in caring for co-occurring mental health conditions.
- Experience with field-based medical outreach.
- Bilingual in Spanish/English.
- Ability to drive a personal vehicle; current Washington State driver’s license and insurable driving record.
Lifting and carrying up to 25 pounds, operating computer hardware; requires close vision, distance vision, peripheral vision, depth perception.
