Position Summary:
Position responsible for identifying, triaging and co-managing patients with medical and behavioral health problems within the primary care setting by providing the Primary Care Behavioral Health (PCBH) model. The PCBH model provides population-based care services within primary care. The Behavioral Health Consultant (BHC) should: be a Generalist; be Accessible; be a Team Player; be highly productive in the number of patient visits per day;
be an Educator to patients and the team about behavioral health needs; and be a routine part of patient healthcare.
Compensation is dependent on skills and experience.
Your contribution to the team includes:
- Collaborate with primary care providers and other team members in recognizing and treating mental health conditions, substance use/misuse, subclinical presentations of distress, psychosocial problems, promoting preventive medicine interventions, chronic condition management, and other behavioral needs of our patient population.
- Provide population-based behavioral health care within the clinic, including establishing behavioral health services as routine patient care, triaging during teachable moments (warm handoffs), balancing scheduled visits with warm handoff opportunities, and using brief assessments/screeners to gauge functioning and risk; work as a generalist with patients of all ages and clinical needs.
- Refer cases to mental health specialists as appropriate and bridge care until connection with traditional mental health services is achieved.
- Provide brief behavioral health visits in line with primary care pace; typical behavioral health visits are about 30 minutes; warm handoff visits may vary 15–30 minutes. Schedule follow-ups based on clinical need while maintaining access to services.
- Communicate effectively with team members about patient care and provide consultation on assessments and interventions.
- Educate patients and families on evidence-supported interventions to improve self-care, quality of life, and functioning; opportunity to present to staff on behavioral health topics.
- Conduct, design and co-lead group classes and group visits to promote skill building and health.
- Monitor the site’s behavioral health program, identify problems in patient services, and make quality improvement recommendations.
- Attend site and quality improvement meetings as required; participate in evaluation of peers and support staff; assist in growing Behavioral Health services.
- Assist in evaluating the behavioral health program: review utilization data, establish outcomes, and assess cost effectiveness of population-based programming.
- Support the Behavioral Health Education Program by mentoring trainees and providing some supervision as needed, within licensing constraints.
Must have’s you’ll need to be successful:
- Doctoral degree in Psychology.
- Washington licensed or license eligible; maintain current registration/licensure in Washington.
- Experience in a health care/medical setting is required; experience with adolescents is required.
- Ability to write reports and complex documents; ability to present to diverse audiences.
- Ability to communicate effectively with adolescent populations and school personnel.
