Pay Range

This range is provided by International Community Health Services. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.

Base pay range

$25.00/hr - $36.60/hr

Job Summary

The Assisted Living Program Coordinator assists the Wellness Nurse Manager to lead the activities of the Resident Services Assistants team to ensure seamless high quality service delivery to Assisted Living Program residents. This role is the liaison between the residents’ families, PACE team and AL care team for the RA schedule coverage and residents care coordination. The role is responsible for promoting the mission of quality of life, dignity, and choice for all residents.

Education
  • HS/GED required.
  • Active CAN, or HCA, or SW certificate preferred.
Experience
  • Two (2) years of working with the frail or elderly preferred.
Other Requirements
  • CPR/First Aid (within 30 days).
  • Food Handle Permit (within 30 days).
  • Long Term Care Worker Basic Training (within 120 days).
  • Nurse Delegation Certifications preferred (within 120 days), or willing to obtain within specified days after hiring.
  • Bilingual language in Chinese preferred.
