Competitive Benefits Package
  • Pay range: $58,400 to $63,100 annually (may increase based on experience).

Essential Job Functions
  • Develop and implement person‑centered, holistic care plans; conduct comprehensive in‑home assessments and re‑assessments.
  • Identify clients’ needs and facilitate access to community resources, manage referrals, and monitor outcomes.
  • Authorize services and oversee Medicaid funding (including Provider One). Communicate changes and terminations to clients.
  • Advocate for clients in obtaining services across domains such as citizenship, housing, food, medical resources, legal aid, and financial eligibility.
  • Assist clients with transitions to long‑term care settings; coordinate placement with agency and community providers.
  • Report required cases (e.g., suicidal ideation) to Adult Protective Services and County crisis responders.
  • Provide short‑term crisis intervention and assist with home safety modifications.
  • Maintain timely documentation, participate in quality assurance, and meet productivity standards.
  • Attend agency, program, and team meetings and trainings.
Position Qualifications
  • Education & Experience:
    • Master’s degree in social work, behavioral, or health sciences with bilingual or bicultural skills; OR
    • Bachelor’s degree in the same fields with bilingual or bicultural skills; OR
    • Associate degree with one year of paid social‑services work and bilingual or bicultural skills; OR
    • High school diploma plus three years of paid social‑services work and bilingual or bicultural skills.
    • Completion of a Case Manager training program including state‑sponsored CORE Training (beneficial).
  • Cultural Humility:
    • Effectively communicate with diverse backgrounds and cultures.
    • Provide respectful, quality care responsive to cultural beliefs and practices.
    • Fluency in an Asian or Native Hawaiian/Pacific Islander language preferred; multilingual skills highly valued.
  • Knowledge and

    Skills:

    • Customer service, empathy, compassion, patience.
    • Time management, problem solving, adaptability.
    • Conflict resolution to mediate between clients and caregivers.
    • Familiarity with aging, community service delivery, and Title XIX case‑management programs.
    • Understanding of adult illnesses and impact on daily tasks.
    • Judgment regarding safe environments during client assessments.
  • Technical

    Skills:

    • Proficiency with Microsoft Office, Outlook, Teams, Zoom, and related web applications.
    • Internet research and data‑entry skills.
  • Additional Requirements:
    • Reliable transportation and a valid driver’s license (personal vehicle must be insured).
    • Adherence to all agency policies and procedures.
Working Conditions
  • Hybrid schedule: 60% on‑site (home visits) and 40% remote.
  • Regular mobility, sitting, and occasional lifting up to 20 pounds.
  • Noise level moderate.
