** Our Support Service Professionals:*
* · Provides information to referral sources and potential clients by receiving intake inquiries and giving tours;
· Identifies clients’ service needs by gathering and completing intake information and working with the client, caregiver and/or referral source to develop service plans;
· Maintain accurate records on housing search process for verifiable billing. Document and communicate with clients, agency and community partners on housing search progress.
· Creates and attend networking events, industry conferences, and community gatherings to expand your network.
· Trains new team members as they gain experience to support the sales effort.
· Reinforces the community’s brand reputation and achieve maximum occupancy goal.
· Develop a person-centered plan and assessment to meet the needs of community integration to include short and long-term goals as well as goal-driven action plans to address client needs and concerns.
· Provide case management and create individualized care plans to meet the needs of our clients
** What you will need to succeed!*
* · Master’s degree in social work, gerontology, or other human services field, or counseling and at least one year of professional work experience (full-time equivalent), or a bachelor's degree in social work, counseling, or a related field and two years of experience in a human services field, or related field.
· Must complete the HIV/AIDS information course;
· Must receive an annual influenza vaccine unless granted a medical or religious exemption.
** Physical Requirements for the role:*
* · Must be able to lift 25 lbs. and support clients’ mobility;
