The Physical Therapist – Home and Community Program aims to make a meaningful impact in the environments that matter most—clients’ homes, workplaces, and communities. We’re looking for an enthusiastic Physical Therapist passionate about helping individuals build independence through personalized care and real‑life experiences.

• Provide therapy in natural settings to support functional goals.
• Help clients participate in everyday activities that promote independence and quality of life.

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
• Communicate patient’s needs and progress to the treatment team, physician, and family members.
• Coordinate with other disciplines to ensure collaboration, coordination of care, and enhance patient outcomes.

• Minimum of bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as a Physical Therapist in the state of practice.
• Current Basic Life Support or Cardiopulmonary Resuscitation certification in accordance with state regulations.
• One year’s work experience as a physical therapist preferred.
• Knowledge of rehabilitation techniques related to complex neurological injury.
• Effective and professional verbal and written communication skills.
• Ability to lift 50 pounds and to transfer or move patients up to 100 pounds following safety procedures.
• Fine motor skills, visual acuity, and capacity to walk/stand for extended periods.
• Physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning.
• Ability to undergo a health screen or examination as required by policy and regulations.
