Hours: Phone Screening: M-F, 7:30am-6pm. Drop-in Assessment: M-F, 8:30am-2pm.
Fee: Sliding scale. WA Apple Health (Medicaid), Medicare, and private insurance are accepted. Those on Medicare must also be receiving WA Apple Health (Medicaid). Insurance panels, depending on the site, are often full.
