• Comprehensive Diagnosis/Treatment Planning.

• Crowns/Bridges.

• Dental Doctorate Degree.
• Current Malpractice Policy.
• Active DEA License.
• Current CPR/BLS Certification.
• National Provider Identifier number (NPI).
